PHP with JS chained select

Hi, I'm currently using the chained select contribution from dynamic drive. The following code below is captured from a JS file. What I would like to do is to output all these constant readings from mySQL (using PHP) but I cannot find the right way from a loop to do it. Note: Each lines that contains: "", "", 1 and everywhere it says to select an option must NOT be looped. Anybody has an idea on this one ?

You can output PHP code as a javascript, but need to escape it correctly.
